Quality and accreditation systems are of interest in laboratory medicine. The author of this book was involved in the early developments of the Kings' Fund Hospital Organisational Audit Programme, in the establishment of Clinical Pathology Accreditation (UK) Ltd and in the preparation of Essential Criteria for Quality Systems of Clinical Biochemistry Laboratories in the EU by the Working Group on Harmonisation of Quality Systems and Accreditation of the European Communities Confereration of Clinical Chemistry. The aim of the book is twofold. This text firstly introduces the reader to quality and accreditation systems and secondly, provides a source of practical information for pathology laboratories preparing for accreditation and perhaps trying to seek accreditation under more than one system. The opening chapters discuss the nature of accreditation, standards and approaches to preparing for accreditation. The main systems referenced throughout are the quality standards ISO 9001:1994, the European standard for testing laboratories EN 45001:1989, the OECD Principles of GLP and the pathology programmes run by the College of American Pathologists and Clinical Pathology Accreditation (UK) Ltd. The next five chapters deal with documentation including a procedure for the preparation and control of documents; the management issues of organization, staffing, facilities, equipment; and health and safety. The book continues with chapters on operating processes, pre- and post-analytical as well as analytical, and include the management of purchasing, computing and near-patient testing. The final chapters cover quality assurance and evaluation, including a proposed structure for quality assurance activities, the process of inspection and the granting (and responsibilities) of accreditation. The book also includes figures giving the comparative requirements in different quality and accreditation systems, and contents pages of procedures and material from the realistic but fictional pathology laboratory of St Elsewhere's Hospital Trust.
